How do I make a referral to geriatrics / rehabilitation?
- Fill out the referral form / consult form. It should be in an A4 sized pad near the ward clarks of each ward.
- Either ask the ward clark to fax across, or fax to this number : 9616 8461
- Important: You need to add a 0 in front of all fax numbers when faxing. ie: 096168461
- Put the original form in the front of the folder
- The geriatrics / rehabilitation consultant will come around a few times a week to consider TOC’s. Their decision / consult will be written in the original form
- Hint: If hand-writing is difficult to read, go to the last line. If it says “… 1B” then your TOC was likely successful.
How do I figure out who’s on take?
- Call switch OR
- Go to intranet -> rosters -> VMO.
How do I make a referral to LVH for the specialties outside of the ones at FFH?

How do I make a rheumatology consult?
Unfortunately, FFH does not offer our own rheumatology specialty service. Furthermore, LVH rheumatology does not give phone advice to FFH.
- Ask for advice from the gen med consultant on take OR
- Fax a normal FFH referral form across to rheumatology clinic for outpatient follow-up.
